This policy explains how the Child and Family Support Services, Inc. website handles information. It covers this website only. How CFSS protects your health information as a patient is described in our Notice of Privacy Practices.

Information you choose to share

If you use the “Request a call back” form, we collect your name, phone number, email address (if you provide one), your preferred office, language, and time to call. We use it only to call you back. The form does not ask for health information, and we ask you not to include any.

Requests are stored in a private database that only authorized CFSS staff and our website administrators can access.

Email

Email is not a secure way to send health information. Please call the office for anything related to your care.
